After the insured event you must notify JSIC Seesam Latvia as soon as possible but not later than in 30 (thirty) days.

What to do in case of an insured event:

  1. Visit your doctor and follow his instructions.
  2. Inform JSIC Seesam Latvia as soon as possible. 

In order to receive insurance indemnity, you must submit the following documents:

  • Health insurance claim.
  • Original cash register receipt or accountable cash payment receipt (specifying the date, number and paragraph of the SRS order  permitting operations without using cash register receipts) containing the following information:

    • Service provider's name, registration number, legal address.
    • First name, surname, identity number of the service recipient- insured person.
    • Service reception date.
    • Description (detailed description) and number of services received.
    • Price of the service received.
    • Sports activities season ticket: validity period of the season ticket must be specified in the documents confirming the payment.
    • Extract from out-patient and/or dental patient and/or in-patient medical record that attests the received services and specifies full diagnosis, treatment used, and tests made and confirming the diagnosis.
    • A copy of the prescription must be submitted in case of medicines or optics which according to the health insurance program must be sold against prescriptions only.
    • If the insurance indemnity is received by an authorized representative, a notarially certified power of attorney must be presented.
    • Other documentation requested by the insurer as additional information to evaluate the health insurance claim and occurrence of an insured event.
